After you have confirmed your appointment with us, please bring the following documents for your visit:

Passport

Ensure that the validity of your passport should not be less than 6 months* and that you have obtained a valid visa to travel to Malaysia, where applicable.

Emergency Contact*

Provide complete contact details of your next of kin in the event of an emergency for us to reach out.

Past Medical Records**

If you have had earlier consultation or tests done with other doctors, please bring along your medical records when you come to the clinic.

Current Medications

Please bring along all medications that you are taking / using at the moment.

Doctor’s Referral Letter**

If you have been referred by a doctor for a particular treatment, please bring along the referral letter.

Insurance Card**

If you have an insurance card issued in Indonesian or any other language, don’t forget to also bring it with you.

Insurance Form**

If you have an insurance form to be completed, kindly complete form accordingly before your visit.

Guarantee Letter**

If the cost of your doctor’s appointment is covered by your company, bring the guarantee letter with you when you come for your visit.

For your safety and comfort, we highly advise that you come for your appointment accompanied by someone.
All treatment will be on outpatient basis.
Please check with your insurance agent before your appointment whether your treatment is covered under your insurance policy.
